Private Health Strategy Assessment — Service & Purchase Terms

The service

The Private Health Strategy Assessment is a structured health-coaching engagement provided by Klein Health Coaching.

Current investment: US$1,250.

It ordinarily includes:

• a detailed pre-assessment questionnaire;
• review of relevant information voluntarily supplied by the client;
• a 90-minute Private Health Strategy Session;
• KHC coaching synthesis;
• a written Private Health Strategy, normally approximately 3–6 pages; and
• a 30-minute debrief.

The Assessment is a standalone service.

It is not a sales call and does not require a client to purchase ongoing KHC coaching.

The appropriate conclusion may be to continue with KHC, implement independently, consult another professional, defer coaching, or take particular questions to an appropriate healthcare professional.

Coaching, not medical care

The Assessment is a health and wellness coaching service.

It is not:

• a medical examination;
• a diagnostic work-up;
• medical treatment;
• emergency care; or
• a replacement for appropriate healthcare.

KHC does not guarantee a particular health or medical outcome.

Application and acceptance

Submitting an application does not necessarily create a coaching relationship.

KHC may decline or defer an Assessment where coaching does not appear appropriate, a need falls materially outside coaching scope, professional healthcare input should occur first, suitable capacity is unavailable, or another professional appears more appropriate.

If KHC has received payment for a service that it subsequently declines before providing it, the amount paid for the unprovided service will be refunded.

Payment

Unless expressly agreed otherwise, payment is due before the Assessment begins.

Cancellation and consumer rights

Nothing in these terms removes any cancellation, withdrawal, refund or other consumer right that applicable law says cannot lawfully be waived.

For qualifying distance-sale services, applicable law may provide a statutory cancellation period and may permit proportionate payment for service already supplied when a valid cancellation occurs after performance has begun.

Applicable statutory cancellation fees may apply where legally permitted.

Rescheduling

Please provide at least 48 hours' notice when requesting to reschedule a private session.

With at least 48 hours' notice, a session may normally be rescheduled without charge.

A request made less than 48 hours before a session, or failure to attend, may result in the session being treated as used, subject to mandatory consumer law and reasonable discretion for genuine emergencies or exceptional circumstances.

Cancellation by KHC

If KHC must cancel a session, a reasonable alternative will be offered.

If KHC cannot provide a material paid part of a service, an appropriate refund for the unprovided portion will be made.

Client information

Clients should provide information they reasonably believe to be accurate and should supply only information reasonably relevant to the coaching work.

KHC may decline to review information that falls outside coaching scope or is unnecessary.

Supplying a medical document does not convert the Assessment into medical care.

Intellectual property

The written Private Health Strategy and other individualized KHC materials may be used by the client for personal purposes and may be shared with their healthcare professionals.

KHC retains ownership of its underlying methodologies, frameworks, templates and general intellectual property.

Ongoing coaching

If continued coaching appears useful, KHC may recommend the 12-Week Private Health Strategy & Implementation engagement or another appropriate service.

No further service is automatic.

The current 12-week engagement investment is US$7,500 when offered.

Privacy

Personal information is handled in accordance with the KHC Privacy Policy and applicable law.

Health information should be supplied through the process KHC designates for that purpose rather than through general public website forms.
