Assess

Start with goals, context, existing healthcare, behaviors, constraints and relevant information.

Prioritize

Use importance, evidence, modifiability, leverage, feasibility and useful feedback to decide what deserves attention. Normally one to three active priorities.

Implement

Convert priorities into specific actions, cues, environment design, fallbacks and realistic frequency.

Measure

Separate process measures from outcomes. Track only what can change a decision.

Learn

Treat setbacks as information about plan design, time, environment, energy, skill or competing priorities—not as a moral failure.

Adapt

Keep, simplify, modify, pause, escalate or refer, and add or replace only when appropriate.

Consolidate

Build routines that can survive work, travel, family demands and imperfect weeks.

Continue or graduate

Independence is a success criterion. Paid support continues only when there is a genuine ongoing job to be done.
